Worker self-registration application

Apply to register for portable long service leave if you are a worker in the community services, contract cleaning or security industries in Victoria.

To find out if you’re already registered for the Scheme or if you have forgotten your Worker ID, you don’t need to fill in this form. Your employer will be able to confirm your registration and provide your Worker ID. If your employer is unable to provide this information you can contact us on 1800 517 158(opens in a new window) or email enquiries@plsa.vic.gov.au(opens in a new window).

Use this form if you think you are covered by the Scheme and your employer is not registered with us (check using our find your employer tool) or doesn’t think you are a covered worker.

We will review the information and contact you with any further questions.

Contract cleaning

Are you employed to clean premises? Swimming pools are premises. Motor vehicles, including RVs and caravans, are not premises.

The Portable Long Service Authority (the Authority) collects the information that you provide with this registration form. The information that you provide may be used to determine your registration under the Victorian Portable Long Service Scheme and follow up your registration request, as per the Long Service Benefits Portability Act 2018. Providing incorrect information may delay the registration process or invalidate the application, you can request access to, and corrections of, any information provided in this form. Requests for access or correction should be sent to enquiries@plsa.vic.gov.au. The Authority is using the services of Drupal 9 to administer this form. The information that you provide is stored in our Drupal 9 content management system and Microsoft Azure servers.
